Vaccination Information for Healthcare Workers

Please see our vaccine distribution phases to find out who is currently eligible to receive COVID-19 vaccine. It will be some time before vaccine is available to the general population.
If you are an eligible healthcare professional who lives or works in Contra Costa County, please use this online form to submit an appointment request through the County. After we review your request, we will contact you with further instructions on scheduling using the MyChart website. Only the healthcare professional who makes the appointment will be eligible to receive the vaccine.
At this time, appointments can only be made online through MyChart but a dedicated vaccine appointment phone line is being established. There are currently limited appointments available. We encourage eligible people we have contacted to check MyChart every morning for newly available slots. Click here to view MyChart tip sheet.
Healthcare workers who have coverage through Kaiser Permanente should schedule a vaccination appointment through Kaiser.

Healthcare Workers
Frontline healthcare personnel can now receive COVID-19 vaccine through their employers or the County.

Hospitals: Staff at the eight acute-care hospitals in Contra Costa can receive COVID vaccine though their employers. Contact your employer for specifics.

Paramedics: The County has scheduled vaccination clinics for paramedics, including firefighters who are paramedics, through the first week of January. Contact your employer for more information.

Dialysis Centers: The County has scheduled several vaccination clinics for staff at dialysis centers. Contact your employer for more information.

In-Home Support Services: The County is in the process of identifying and contacting these healthcare workers. IHSS workers may also use this online form to submit a request for a vaccination appointment. We will contact you with further instructions.

Primary Care Clinics: The County has begun contacting licensed primary-care clinics to help plan for providing vaccines to these sites. Frontline staff at primary care clinics can also directly schedule vaccination appointments online through the County (see above).

Dental and Oral Health: As of 12/30/2020, employees in this healthcare sector are now eligible for vaccination.

Laboratory Workers: As of 12/30/2020, employees in this healthcare sector are now eligible for vaccination.

Specialty Clinics: As of 12/30/2020, employees in this healthcare sector are now eligible for vaccination.
Nursing Homes
Staff and residents of long-term care facilities (e.g. skilled nursing facilities and residential care facilities for the elderly) are now eligible to receive vaccine. These vaccinations will be managed by the Pharmacy Partnership for Long-term Care (LTC) Program, a federal partnership with CVS and Walgreens, unless otherwise noted below.

Skilled Nursing Facilities: The federal LTC program has begun scheduling vaccination clinics at SNF’s in Contra Costa. Any staff working in skilled nursing facilities may also use this online form to submit a request for a vaccination appointment.

Residential Care Facilities for the Elderly: The County is currently coordinating vaccinations at more than 40 of these facilities in Contra Costa. Staff working in these facilities may also use this online form to submit a request for a vaccination appointment.
Behavioral Health

Residential Mental Health & Substance Use Disorder Treatment: The County is coordinating vaccinations for staff at these sites. Staff working in these facilities may also use this online form to submit a request for a vaccination appointment.

Outpatient Mental Health & Substance Use Disorder Treatment: The County is currently surveying county behavioral health staff and those working for contracted community partners. Staff working in these facilities may also use this online form to submit a request for a vaccination appointment.
